When scheduling an appointment, please also register with us as a user by sending us the Patient Registration Form. While this is optional, it takes only a few minutes and will save you time when you visit us.

What to bring to an appointment

First Time Patients
Please make sure you have the following when you visit us:

  • Insurance Card(s)
  • Picture ID
  • Copayment (if applicable)
  • Medical or Personal Records, such as X-rays
  • A list of your current medications (if any)

Returning patients

Helpful Tips

Give yourself an extra 15 minutes before your scheduled appointment for parking and getting to your appointment.
